Investigators with "The Centers for Medicare and Medicaid Services" visit homes in an effort to identify deficiencies in care.
Late last year, "Golden Cross Health Care" was added to the list of homes in need of close monitoring.
Investigators included "medication distribution" among the issues needing improvement.
The nursing home's new administrator says the facility's owner brought in new managers last month and they are working to fix the problems.
